Documents Required

One primary and two secondary documents are required for a driver’s license in most states.

Primary Documents

1.Birth Certificate written in English or attached English translation which is notarized
2.Certificate of Naturalization
3.Current out-of-state driver’s license with photo
4.Current military identification card
5.Merchant mariner’s ID with photo
6.Aliens under 18 with foreign Birth Certificate/English translation
7.Current passport only if 18 or older

Secondary Documents

1.I-94 Card
2.Medicare/Medicaid Card
3.Motor vehicle title/registration
4.Original university, college, high school diploma.
5.W-2 for 2 yrs.
6.Voter Registration Card
7.Original divorce, separation, marriage license
8.Student ID card with photo (La. School)
9.Social Security Card
10.Three payroll stubs with SS#
11.Income tax form
12.Military dependent card
13.Original documents of adoption
14.Certified copy of professional degree, license
